Hey Eric -- great post! I definitely agree with many of the points you make, especially the need for social listening and getting the entire team involved in social / community management. Social listening is also a great way to discover new markets you didn't even know were interested in your product.
For social listening tools, you might be interested in mention.com, a media monitoring app that allows you to monitor, react to, and share keyword mentions right from the web or mobile app.

Good stuff man, I agree with everything you are saying. Under 3. Integrating Social Media Company-Wide you mention Product Development, Customer Service, HR, and Marketing. What about sales? Providing sales the ability to leverage social media to create opportunity is HUGE. You can use a listening tool to uncover prospects and with proper targeted content disemination you can drive the prospect over the finish line.
